ORGANIZATIONAL CHART

PROGRAM DESCRIPTIONS

Community-Based Residential Support Services provides support to people who have a disability and live within a community-based home that is maintained by Hiawatha Homes.  Direct Support Professionals/Aides provide support in areas such as: advocacy, community involvement, nursing and personal care support therapies, social and recreational activities.
Typical hours vary between 6a-10a or 6a-2p, 2p-10p or 3p-9p, 10p-6a or 10p-8a.

Family Support Services provides support to youth and adults who have a disability that live in their family’s home or in foster care.  Direct Support Professionals/Aides provide 1:1 support in areas such as: life time skills training, personal needs training, recreational/leisure activities, social activities, and community involvement.
Typical hours may include 6a-7:30a or 3p-6p weekdays and typically no later than 5pm on weekends.
Respite hours available Friday Saturday and Sunday include 6a-2p, 2p-10p, and Sleep Overnight 10p-6a.

*A valid driver’s license, satisfactory motor vehicle record is required for morning and evening shifts and successful completion of Hiawatha Homes driving test.
